As a Mochi Health patient, Mochi may communicate Protected Health information (PHI) with you via unencrypted email. By consenting to receive emails containing PHI, it will allow us to do things like sending you detailed appointment confirmations and reminders and provide you with status updates on your prescription medications by email. Please read this agreement, which informs you of the potential risks of communicating PHI over email. By agreeing to this policy, you authorize Mochi Health to send you emails that may contain Protected Health Information (PHI) under HIPAA.
Important Disclosures
Email communication may not be encrypted. Unencrypted emails may be intercepted by unauthorized parties during transmission. This means your PHI could be read by unintended recipients if intercepted.
Emails may be accessed by others - if you share a device with other people, if your email address is provider through your employer, if you leave a device unattended, or if you otherwise allow another person access to your email, your emails containing PHI may be accessed by someone who you do not wish to know about your health information. Despite reasonable precautions, emails may be sent to the wrong address by either party. Messages and attachments can be read, copied, and forwarded by anyone with access to the email account or if the email is accidentally sent to the wrong recipient.
Emails may be difficult to verify - Email may be easier to forge than handwritten or signed papers.
Mochi Health is not responsible for unauthorized access to your information that occurs due to email limitations beyond our control.
You can opt out at any time.
