Introduction and Agreement. You authorize Mochi Health Corp. (“Mochi”) to use and disclose your protected health information (PHI) as described herein (the “Consent”) for the marketing purposes outlined below. This Consent is voluntary. The date and time of your acceptance will be captured electronically. Your treatment, payment, enrollment, or eligibility for benefits with Mochi is not conditioned on your agreement.

Mochi does not sell PHI to any third parties. Your PHI is used exclusively for Mochi’s business purposes by Mochi and by its business associates acting on our behalf.

Information Used/Disclosed. Mochi may use and disclose the minimum necessary PHI relevant to the marketing purposes described here, including your name, contact information; demographic details such as age range and ZIP code; enrollment and service history with Mochi; clinical information relevant to offerings being marketed (for example, diagnoses, conditions, medications, or lab values); and billing or claims information.

Who May Use/Disclose and To Whom. You authorize Mochi and its business associates acting on its behalf to perform the marketing activities described in this authorization.

Use. By agreeing to this Consent, you agree that Mochi may use your PHI to market its products, services, and offerings to you and to identify individuals who may benefit from or be eligible for Mochi’s products, services, and offerings. Communications with you may be personalized based on your PHI to improve relevance.

Contact Methods. This authorization allows Mochi to contact you using various communication methods, which may include email, SMS/text message, telephone or voicemail, postal mail, in‑app or push notifications, and targeted digital advertising that uses your contact information (for example, hashed email or phone number) shared with advertising platforms acting as business associates. Standard carrier or data rates may apply for SMS.

Your Rights, Revocation, and Copies. You may revoke this authorization at any time by submitting a written request to Mochi at privacy@joinmochi.com or, if you have an active Mochi Health Membership, by using the revocation mechanism provided in your account settings. Revocation will not affect actions already taken in reliance on this authorization. You have the right to receive a copy of this authorization. Mochi may create and use de‑identified information derived from your PHI; de‑identified information is not PHI and is not subject to HIPAA.
