At Mochi Health, we've always believed that technology should make medicine more human, not less. When we set out to build an AI documentation tool for our provider network, we didn't license someone else's solution. We built one from the ground up, designed specifically for how our providers see patients and how our patients experience care.

Today, we're sharing that work publicly for the first time.

The Mochi AI Scribe is a fully integrated AI documentation assistant that lives natively inside our EHR. It listens, understands, and documents so providers can stay present with their patients instead of splitting their attention between a conversation and a keyboard. And with our latest update, it now generates personalized after-visit summaries and patient follow-ups that extend the quality of that care beyond the appointment itself.

AI Medical Scribe with Full EHR Integration

Most AI scribe solutions are designed for in-person clinical settings. They handle transcription well enough, but they come in cold. They don't know who the patient is before the visit starts.

That's a fundamental problem for what our providers do. Practicing obesity medicine at scale means managing complex, longitudinal relationships with patients navigating GLP-1 therapies, metabolic health, and deeply personal lifestyle change. Context isn't background information, it's clinical substance.

The Mochi Scribe is embedded directly into our EHR, giving it access to the full patient chart before a single word of the appointment is spoken: prior visit notes, current medications, dosage history, flagged side effects, goals the patient has been working toward. That context shapes every note automatically, producing documentation that reflects not just what was said today, but where it fits in the patient's broader story.

"I had used other scribe tools before, and they were helpful for transcription, but they never really understood the patient. What impressed me about Mochi's Scribe is that it starts with context. Before the visit even begins, it already knows the patient's history, medication changes, prior side effects, and long-term goals. In obesity medicine, where success depends on continuity and long-term relationships, that context makes a tremendous difference." — Roberto Valledor, MD, Provider with Mochi Health

AI Scribe That Adapts to Your Documentation Style

Documentation style is deeply individual. The way a provider structures a note, the language they use, the level of clinical detail they include, that reflects years of practice. We didn't want providers spending their time editing generic output until it sounded like them. We wanted the tool to learn them.

The Mochi AI Scribe adapts over time to each provider's preferred format, terminology, and documentation cadence. The result is a note that requires minimal editing, because it was already written in their voice from the start.

"One thing that surprised me was how quickly the Scribe adapted to my documentation style. Within a short period of time, the notes felt like they were written the way I naturally practice medicine. I conduct visits in both English and Spanish, and it performs remarkably well in both languages. Today, I spend far more time reviewing notes than rewriting them, which has saved me hours of documentation every week." — Roberto Valledor, MD, Provider with Mochi Health

For a network of 500+ providers across 50 states, all with different specializations and practice styles, this flexibility isn't a nice-to-have. It's foundational.

After-Visit Summaries & AI Patient Follow-Up

The Mochi AI Scribe solved the in-visit documentation problem. Our latest update extends that into what happens after the visit ends.

Anyone who practices telehealth knows the gap: a patient leaves a video call and returns to their life, and what happens in that window, whether they stay adherent, whether they feel supported, whether they reach out when something comes up, often determines whether treatment succeeds or doesn't. Generic follow-up messages don't close that gap.

Mochi's new after-visit summary and follow-up feature generates a personalized communication for each patient based on the actual content of their encounter — what was discussed, what was decided, what to watch for, and what comes next. It draws from the visit and the patient's full clinical context to produce something that reads like it was written specifically for them.

Because it was.

"As physicians, we know that a large part of successful treatment happens between visits. The personalized follow-ups created by the Scribe reference the actual conversation, recommendations, and goals discussed during the appointment. Patients notice that level of personalization. It helps them feel supported, improves engagement, and reinforces the care plan long after the visit has ended." — Roberto Valledor, MD, Provider with Mochi Health

At the scale Mochi operates — over 200,000 active patients — that level of personalization has historically been impossible to deliver consistently. This feature makes it automatic.

Better documentation is safer documentation. Personalized follow-up improves adherence. And providers who aren't exhausted by administrative burden are better providers. Those aren't soft benefits, they're clinical outcomes.

The Mochi AI Scribe is live across our provider network today. The after-visit summary and personalized follow-up feature is rolling out now. This is what Mochi AI looks like. There's more to come.
