Metformin hydrochloride
Metformin is a first-line oral medication for type 2 diabetes that has been used safely for decades. It works primarily by decreasing glucose production in the liver and improving the body's sensitivity to insulin, without causing low blood sugar on its own. It is typically started at a low dose with meals and gradually increased to minimize gastrointestinal side effects. Extended-release formulations are often better tolerated than immediate-release.

Side effects
Common side effects of Metformin include gastrointestinal issues such as nausea, vomiting, and diarrhea. Some individuals may also experience a metallic taste in their mouth.
Avoid When
Significant kidney disease (eGFR <30); active liver disease; conditions predisposing to lactic acidosis (severe heart failure, severe infection, dehydration, excessive alcohol use); known hypersensitivity. Temporarily hold around iodinated contrast imaging procedures and during acute illness with dehydration. Use caution with vitamin B12 deficiency (monitor periodically with long-term use especially if you are vegatarian).
